The District A-1 Sight Conservation Foundation, commonly called "Eyes Right", is a charitable foundation established by the Lions of District A-1 to contribute to the preservation of eyesight. Through this foundation, the District A-1 Lions Clubs have contributed many specialized devices for eye treatment and diagnosis to several hospitals in Southwestern Ontario. DISTRICT A-1 SIGHT CONSERVATION FOUNDATION "EyesRight" "We serve" So Others can See Eyes right is an on-going project in your district. It is your foundation all funds collected are spent in your district. The foundation is a REGISTERED CHARITY. All funds requiring a tax receipt including bingo and Nevada tickets can be directed to the foundation. Further private donations, memorials (we have cards in most funeral homes) or estate funds will be issued tax reaccepts THE NEED In today’s health care environment hospitals & clinics need assistance to fund new technology/new equipment used in the area of eye care. THE PURPOSE To provide funding to hospitals and/or clinics in DISTRICT A-1 to create "Centres of Excellence" in eye care. To promote the awareness of the need of donated cornea. PAST Your foundation began raising funds in the fall of 1990. With the generous support of Lions, Lioness & Leos, your foundation has purchased and paid for: A Keratascope for L.H.S.C. Ophthalmology Department University Campus($43,800) Anterior Segment Machine for St.Thomas Elgin General Hospital Ophthalmology Dept. ($49,200) A/B Scanner for Chatham/Kent Health Alliance Ophthalmology Dept. ($35,000) Phaco Machine for Sarnia General Hospital Ophthalmology Suite ($65,000) Alcon Laser for Hotel Dieu Grace Hospital Ophthalmology Suite in Windsor ($65,000) Phaco Emulsifier for Strathroy Middlesex General Hospital ($50,000) Orb Scanner for the London Health Sciences Centre ($50,000) Chatham-Kent Health Alliance Project (120,000) We always welcome proposals from a Club(s), Zone or Region. Commit yourself and your Club to the goals of the foundation. Participation in the activities of your Foundation, will have a positive impact on the lives of the visually impaired in your own District, possibly your own community PROPOSALS REQUESTED Is there a need for a PROJECT in your area of District A-1? Use the following criteria to prepare your proposal: * Is Your Club, Zone, Region, and District Governor Aware and Supportive of Your Idea? *Is the Facility Receiving the Benefits of Your Proposal Aware of and Supportive of Your Idea? *Does the Facility Benefiting Form Your Proposal Have a Fund Raising Foundation of its Own? *Is Their Foundation Aware and Supportive of Your Idea? *Is the Medical Staff Aware and Supportive of Your Idea? *Can You Provide Complete Detail Regarding: *Total Cost (Can. Funds) *Technical Specifications *Financial Arrangements Needed *Statistical Data on Projected Number of People Served Arrange a time to present the proposal to the board of directors. Ask the medical and hospital foundation personnel to accompany you. All of the above details are necessary to prevent duplication of fund raising. The more information the better to help the board make informed and wise decisions.
